Mediastinal widening on CXR is defined as width of more than 8 cm on posteroanterior view. It can be commonly due to lymph node enlargement, vascular causes, neoplasia, and rarely due to gastrointestinal causes such as achalasia or hernia.

What can cause mediastinal widening?

• Thoracic aortic aneurysm of the ascending and proximal descending aorta.
• Aortic dissection of ascending and proximal descending aorta.
• Unfolding of the aorta.
• Traumatic aortic rupture.
• Hilar lymphadenopathy either infectious or malignant.

Why does aortic dissection cause mediastinal widening?

Widened mediastinum carries a broad differential, but one in the setting of atraumatic chest pain should raise the concern for acute aortic dissection. The widening is due to the presence of a new dissecting false lumen next to the true lumen of the aorta.

How do you assess wide mediastinum?

If the mediastinum is still widened or an upright film cannot be performed, obtain a chest CT angiogram with cuts through the aortic arch. If the CT scan shows no periaortic mediastinal blood, or dissection flap within the lumen of the aorta, then an aortic injury has been ruled out.

What is a mediastinal abnormality?

Common symptoms of mediastinal disease include cough, chest pain, and dyspnoea, as well as symptoms relating to any structure being compressed such as dysphagia, stridor, or SVCO. Mediastinal disorders can also be asymptomatic. They may be found incidentally following a CXR.

Widened Mediastinum : Chest X-ray Interpretation

Why is the mediastinum important?

The mediastinum houses many vital structures including the heart, great vessels, trachea, and essential nerves. It also functions as a protected pathway for structures traversing from the neck, superiorly, and into the abdomen, inferiorly.

What does normal mediastinum mean?

The mediastinum is the area in the chest between the lungs that contains the heart, part of the windpipe (the trachea), the esophagus, and the great vessels including the ascending aorta (the large artery which carries blood from the left ventricle of the heart on its way to the rest of the body) and right and left ...

What CXR projection is associated with magnification of the heart and widening of the mediastinum?

Superior mediastinum:

Should have a width <8 cm on a PA CXR. A widened mediastinum can be associated with: AP CXR view, which magnifies the heart and mediastinal structures.

Can Pneumomediastinum cause pneumothorax?

Other complications of pneumomediastinum include extensive subcutaneous emphysema or pneumothorax, which usually require minor interventions, such as skin incisions and chest tube drainage.

What is superior mediastinum?

The superior mediastinum is the region sandwiched between the pleural sac on each side above an imaginary line from the angle of Louis in front to the fourth/fifth thoracic intervertebral disc behind.

What does widening of the chest mean?

Widening of the mediastinum is most often due to technical factors such as patient positioning or the projection used. Rotation, incomplete inspiration, or an AP view, may all exaggerate the width of the mediastinum, as well as heart size.

What is the difference between aortic dissection and aortic aneurysm?

Aortic aneurysm and aortic dissection

Having an aortic aneurysm increases the risk of a tear in the aortic lining (aortic dissection), as shown in the image on the right. An aortic dissection is a serious condition in which a tear occurs in the inner layer of the body's main artery (aorta).

What is LOEY Dietz syndrome?

Loeys-Dietz syndrome (LDS) is a disorder that affects the connective tissues of the body and increases the patient's risk of aneurysms in arteries such as the aorta. This condition is rare and was only recently identified as a condition in 2005.

What is mediastinal?

Listen to pronunciation. (MEE-dee-uh-STY-num) The area between the lungs. The organs in this area include the heart and its large blood vessels, the trachea, the esophagus, the thymus, and lymph nodes but not the lungs.

What are mediastinal masses?

Mediastinal tumors are growths that form in the area of the chest that separates the lungs. This area, called the mediastinum, is surrounded by the breastbone in front, the spine in back, and the lungs on each side. The mediastinum contains the heart, aorta, esophagus, thymus, trachea, lymph nodes and nerves.

How do you fix pneumomediastinum?

Supportive treatment helps treat pneumomediastinum. Your doctor may put you on oxygen and bed-rest to help with healing. This will also help your body reabsorb the air that has leaked. The pain may be managed using analgesics or other drugs.

What is Hemothoraces?

A hemothorax (plural: hemothoraces), or rarely hematothorax, literally means blood within the chest, is a term usually used to describe a pleural effusion due to accumulation of blood. If a hemothorax occurs concurrently with a pneumothorax it is then termed a hemopneumothorax.

Does pneumomediastinum go away on its own?

Spontaneous pneumomediastinum often improves on its own. Once the condition goes away, it usually doesn't come back. However, it can last longer or return if it's caused by a repeated behavior (such as drug use) or an illness (like asthma). In these cases, the outlook depends on the cause.

What is a 2 View CXR?

Typically, two views of the chest are taken, one from the back and the other from the side of the body as the patient stands against the image recording plate.

What is CXR test?

A chest X-ray is a test that looks at your heart, lungs and bones. Chest X-rays use a small dose of radiation to create a black-and-white image. Healthcare providers can look at this image to diagnose and treat broken bones, heart conditions and lung problems.

What is mediastinum in chest xray?

The mediastinum is a space in the thorax that contains a group of organs, vessels, nerves, lymphatics and their surrounding connective tissue. It lies in the midline of the chest between the pleura of each lung and extends from the sternum to the vertebral column.

What occurs when a patient experiences a mediastinal shift?

The build-up of pressure in the pleural cavity causes the mediastinum, which contains the heart, trachea, esophagus and great vessels, to shift to the unaffected side (see Fig. 1). This mediastinal shift also causes compression of the lung on the unaffected side.

What are Hila?

Hila, or lung roots, are relatively complicated structures that consist mainly of the major bronchi and the pulmonary arteries and veins. The hilum of the lung is found on the medial aspect of each lung, and it is the only site of entrance or exit of structures associated with the lungs.

Does the mediastinum contains the pericardial cavity?

Mediastinum - Space between the left and right Pleural Cavities. Contains the Pericardial Cavity which surrounds the Heart, Trachea, Esophagus, Thymus, and Blood Vessals.
