Anticholinergic agents are contraindicated in patients with primary glaucoma, a tendency toward glaucoma (narrow anterior chamber angle), or adhesions (synechiae) between the iris and lens, as well as for the elderly and others in whom undiagnosed glaucoma or excessive pressure in the eye may be present.

Why is atropine not used in glaucoma?

One of the controversial side effects of atropine is its anti-cholinergic effect that leads to drug-induced acute angle closure glaucoma [10].

Is atropine used to treat glaucoma?

Atropine induces cycloplegia by paralyzing the ciliary muscles, whose action inhibits accommodation to allow accurate refraction in children, helps to relieve pain associated with iridocyclitis, and treats ciliary block (malignant) glaucoma.

How does atropine worsen glaucoma?

Antimuscarinics such as atropine are contraindicated in angle-closure glaucoma because of the increased likelihood of producing complete obstruction of the outflow of aqueous humor, resulting in an acute increase in intraocular pressure (IOP) in response to relaxation of the ciliary muscle.

Why are anticholinergics contraindicated in glaucoma?

Anticholinergics inhibit parasympathetic nerve impulses by selectively blocking the binding of the neurotransmitter acetylcholine to its receptor in nerve cells. This can cause glaucoma in individuals with narrow anterior chamber angles by dilating the pupil and causing pupillary block.

Development of Glaucoma Animation, Open Angle vs Angle Closure Glaucoma.

How is atropine metabolized?

Atropine is well absorbed after intramuscular administration. Atropine is distributed throughout the various body tissues and fluids. Much of the drug is metabolized by enzymatic hydrolysis, particularly in the liver; from 13 to 50% is excreted unchanged in the urine.

Can SSRIs cause glaucoma?

Glaucoma risk by exposure to SSRIs

The average dose of SSRIs (i.e. ≥1DDD) was significantly associated with an elevated risk for glaucoma (aOR = 1.13; 95% CI = 1.03–1.24). SSRI cumulative duration of >365 days was also associated with elevated risk for developing glaucoma (aOR = 1.25; 95% CI = 1.04–1.50).

Why is ipratropium contraindicated in glaucoma?

They concluded that whilst it is relatively safe to use nebulized ipratropium bromide and salbutamol separately in patients with narrow-angle glaucoma, a combination of the two drugs carries a higher risk of inducing an attack of acute angle closure glaucoma in susceptible patients.

When is atropine contraindicated?

Contraindications: Atropine generally is contraindicated in patients with glaucoma, pyloric stenosis or prostatic hypertrophy, except in doses ordinarily used for preanesthetic medication.

Does atropine lower eye pressure?

Intraocular pressure and pupil diameter were measured prior to and during a 2-day period of treatment with 1% atropine sulfate. No significant changes in intraocular pressure occurred as a result of the treatment with atropine. Pupil diameter increased significantly after atropine was applied.

Why is atropine used in the eye?

Atropine causes the muscles in your eye to become relaxed. This widens (dilates) your pupil so that it will not respond to light. Atropine ophthalmic (for the eye) is used to dilate your pupils when you have an inflammatory condition or in postsurgery situations in which this effect may be helpful.

Is atropine sympathetic or parasympathetic?

Atropine is a competitive antagonist of muscarinic acetylcholine receptors, a group of G-class receptor proteins, blocking the action of acetylcholine and therefore suppressing the actions of the parasympathetic nervous system.

Why atropine and hyoscine are used extensively in ophthalmic practices give reason?

Atropine is used extensively in ophthalmology because of its remarkable ability to not only dilate ones eye, but prolong the effect for several days. In addition to dilation, paralysis occurs but for a shorter period of time. Hyoscine has these same effect but for a more limited duration.

Why is atropine contraindicated in prostatic hypertrophy?

In general, the use of anticholinergic agents is contraindicated in patients with urinary retention and bladder neck obstruction caused by prostatic hypertrophy. Dysuria may occur and may require catheterization. Also, anticholinergic drugs may aggravate partial obstructive uropathy.

Why is atropine contraindicated in myasthenia gravis?

Antimuscarinics (applies to atropine) myasthenia gravis

Because antimuscarinic agents have anticholinergic effects, they are contraindicated in patients with myasthenia gravis. Their use may be appropriate to reduce adverse muscarinic effects caused by an anticholinesterase agent.

Why is pilocarpine used to treat glaucoma?

Ophthalmic pilocarpine is used to treat glaucoma, a condition in which increased pressure in the eye can lead to gradual loss of vision. Pilocarpine is in a class of medications called miotics. It works by allowing excess fluid to drain from the eye.

Who should not use atropine?

severe ulcerative colitis. toxic megacolon. paralysis of the intestines. blockage of the stomach or intestine.

Why is atropine given before surgery?

narcotic analgesics to treat severe pain. medicines used for glaucoma e.g. pilocarpine, carbachol. medicines to treat Alzheimer's disease e.g. rivastigmine and donepezil. muscle relaxants used during an operation.

Why is atropine used as a Preanesthetic agent?

Atropine may be used during pregnancy as a preoperative, preanesthetic agent to reduce salivation and bronchial secretions. Atropine rapidly crosses the human placenta.

Can you use ipratropium with glaucoma?

Ocular complications can follow combined ipratropium bromide and salbutamol nebulization in patients with narrow-angle glaucoma, but can be prevented by using the drugs separately, protecting the eyes and ensuring continued antiglaucoma measures.

What are contraindications of ipratropium?

• closed angle glaucoma.
• blockage of the urinary bladder.
• enlarged prostate.
• an inability to completely empty the bladder.

Does ipratropium cause glaucoma?

Acute angle-closure glaucoma is an uncommon complication of inhaled ipratropium bromide therapy for asthma. All previously reported cases have occurred in hospitalized patients receiving continuing nebulized therapy.

Is amitriptyline contraindicated in glaucoma?

Patients should be advised that taking amitriptyline hydrochloride tablets can cause mild pupillary dilation, which in susceptible individuals, can lead to an episode of angle-closure glaucoma.

What medications can cause eye pressure?

Certain classes of drugs are known to cause eye problems. These include diuretics (water pills), antihistamines, antidepressants, drugs that lower cholesterol, beta-blockers and birth control pills. “Antihistamines often cause a very significant dry-eye syndrome,” Andreoli says.

What is lexapro used for?

Escitalopram is an antidepressant medication that works in the brain. It is approved for the treatment of major depressive disorder (MDD) and generalized anxiety disorder (GAD). Symptoms of depression include: Depressed mood - feeling sad, empty, or tearful.
