Transmural infarcts involve the whole thickness of myocardium from epicardium to endocardium and are usually characterized by abnormal Q waves on ECG. Nontransmural (including subendocardial) infarcts do not extend through the ventricular wall and cause only ST-segment and T-wave (ST-T) abnormalities.

What causes transmural myocardial infarction?

An acute ST-elevation myocardial infarction occurs due to occlusion of one or more coronary arteries, causing transmural myocardial ischemia which in turn results in myocardial injury or necrosis.

What does transmural lesion mean?

A transmural lesion—scar tissue that completely penetrates the heart tissue from the inside to the outside—is key to a successful ablation by preventing errant electrical signals. Making a transmural lesion is as much of an art as it is a science.

What is transmural infarction?

A transmural myocardial infarction refers to a myocardial infarction that involves the full thickness of the myocardium. It was one believed that the development of Q waves indicated the infarction was “transmural;” however, autopsy studies failed to confirm this.

What does transmural myocardial infarction mean?

Pathologically speaking, a transmural infarct results when myocardial necrosis extends throughout the entire thickness of the myocardium – this is due to complete occlusion of an epicardial coronary artery (i.e. STEMI).

What is acute myocardial infarction?

What are the differences between a transmural MI and an Subendocardial mi?

The transmural type usually consisted of yellowish-brown coagulation necrosis in the center of an infarcted focus and coagulative myocytolysis at the marginal zone. The subendocardial type was characterized by coagulative myocytolysis throughout the entire focus.

What is the difference between STEMI and NSTEMI?

If there is a pattern known as ST-elevation on the EKG, this is called a STEMI, short for ST elevation myocardial infarction. If there is elevation of the blood markers suggesting heart damage, but no ST elevation seen on the EKG tracing, this is known as a NSTEMI.

Which is definitive feature of transmural infarction?

In transmural MI, ischemia in the subendocardium spreads to the epicardium and involves full thickness of the myocardium. In the acute phase, the ECG signs are ST segment elevation. The elevated ST segment may slope upward or be horizontal or dome-shape.

How do you identify a pathological Q wave?

Q waves are considered pathological if: > 40 ms (1 mm) wide. > 2 mm deep. > 25% of depth of QRS complex.

What does ischemia look like on ECG?

The most common ECG sign of myocardial ischemia is flat or down-sloping ST-segment depression of 1.0 mm or greater. This report draws attention to other much less common, but possibly equally important, ECG manifestations of myocardial ischemia.

What does transmural inflammation mean?

Transmural inflammation was endosonographically defined as sectional interruption or loss of the five-layer structure. In 14 of the 37 patients a colectomy was performed. Examination of 3 of the 14 resected specimens revealed inflammation confined to the mucosa.

What is non transmural infarction?

A non-transmural myocardial infarction refers to a myocardial infarction that does not involve the full thickness of the myocardium. It was one believed that the development of Q waves indicated that the infarction was “transmural," however autopsy studies failed to confirm this.

How does myocardial infarction affect ECG?

In a myocardial infarction transmural ischemia develops. In the first hours and days after the onset of a myocardial infarction, several changes can be observed on the ECG. First, large peaked T waves (or hyperacute T waves), then ST elevation, then negative T waves and finally pathologic Q waves develop.

What diagnostic tests usually confirm an MI?

Tests available include: Cardiac Troponin I or Troponin T - which are both very sensitive and specific and are the recommended laboratory tests for the diagnosis of MI. Serial testing is recommended in order to confirm or exclude a rise or fall in troponin concentration.

How long do Q waves last?

Electrocardiographic Monitoring

For MI, Q waves last longer than 0.04 second, and depth exceeds one-third of the R wave (inferior wall MI). For inferior wall MI, differentiate it from right ventricular hypertrophy by axis deviation.

What are the early warning signs of a myocardial infarction select all that apply?

• mild pain or discomfort in your chest that may come and go, which is also called “stuttering” chest pain.
• pain in your shoulders, neck, and jaw.
• sweating.
• nausea or vomiting.
• lightheadedness or fainting.
• breathlessness.
• feeling of “impending doom”
• severe anxiety or confusion.

What causes abnormal Q waves?

Background: Abnormal Q waves (AQW) in the electrocardiogram are commonly ascribed to underlying myocardial infarction (MI). As an imperfectly specific sign of MI, the usefulness of AQW in identifying MI depends on its incidence in the population studied.

What does an abnormal Q wave on ECG mean?

Conclusion: Abnormal Q waves on the admission electrocardiogram (ECG) are associated with higher peak creatine kinase, higher prevalence of heart failure, and increased mortality in patients with anterior MI. Abnormal Q waves on the admission ECG of patients with inferior MI are not associated with adverse prognosis.

What does a pathologic Q wave indicate?

Pathologic Q waves are a sign of previous myocardial infarction. They are the result of absence of electrical activity. A myocardial infarction can be thought of as an elecrical 'hole' as scar tissue is electrically dead and therefore results in pathologic Q waves.

Which are posterior leads on ECG?

• ST segment depression (not elevation) in the septal and anterior precordial leads (V1-V4). ...
• A R/S wave ratio greater than 1 in leads V1 or V2.
• ST elevation in the posterior leads of a posterior ECG (leads V7-V9).

How is anterior wall MI diagnosed?

1. ST segment elevation in the anterior leads (V3 and V4) at the J point and sometimes in the septal or lateral leads, depending on the extent of the MI. ...
2. Reciprocal ST segment depression in the inferior leads (II, III and aVF).

Can a NSTEMI be fatal?

It's a life-threatening emergency. It will show up as an abnormality on an electrocardiogram (EKG). NSTEMI. In this form of ACS, your heart is getting some oxygen but not enough.

Does NSTEMI go to cath lab?

There are Non-STEMIs that need the cath lab now.

What causes a NSTEMI?

The etiology of NSTEMI varies as there are several potential causes. These include tobacco abuse, lack of physical activity, high blood pressure, high cholesterol, diabetes, obesity, and family history.

Why is Subendocardium more prone to ischemia?

Left ventricular subendocardial muscle is the most susceptible region of the left ventricle to ischemia [41] because it can receive its blood (oxygen) supply only during diastole; intramyo- cardial vessels are squeezed shut during systole.
