Bile acid sequestrants or resins are medications for lowering low-density lipoprotein (LDL) cholesterol often called “bad” cholesterol in the blood. They bind to bile acids and prevent reabsorption of bile acids in the intestines.

What are bile resins?

Bile acid resin: Bile acid resins are substances that bind in the intestines with bile acids that contain cholesterol and are then eliminated in the stool. The major effect of bile acid resins is to lower LDL-cholesterol by about 10 to 20 percent. Small doses of resins can produce useful reductions in LDL-cholesterol.

How does bile acid resin work?

How do bile acid sequestrants work? Bile acid sequestrants bind bile acids in the intestine and increase the excretion of bile acids in the stool. This reduces the amount of bile acids returning to the liver and forces the liver to produce more bile acids to replace the bile acids lost in the stool.

What drugs are bile acid sequestrant?

• cholestyramine.
• colesevelam.
• Colestid.
• colestipol.
• LoCholest.
• Prevalite.
• Questran.
• Questran Light.

What is bile acid resin therapy?

Bile acid sequestrants are medicines that help lower your LDL (bad) cholesterol. Too much cholesterol in your blood can stick to the walls of your arteries and narrow or block them. These medicines work by blocking bile acid in your stomach from being absorbed in your blood.

How do Bile Acid Sequestrants Work? (+ Pharmacology)

What does a bile binder do?

The main type of medication used to treat BAM is called a bile acid binder. It binds with the bile acids in your digestive tract, which reduces their impact on your colon. Bile acid binders are typically very effective at treating diarrhea associated with BAM.

What are the side effects of bile acid?

Since bile acid sequestrants are not absorbed in the gastrointestinal tract, they have limited systemic side effects. Some of the most common adverse effects are gastrointestinal, including constipation, stomach pain, bloating, vomiting, heartburn, loss of appetite, indigestion, and upset stomach.

What are the symptoms of bile acid malabsorption?

• watery diarrhoea — often yellow and greasy in appearance.
• needing the toilet often.
• bloating.
• abdominal cramps.

Does colestipol cause constipation?

Constipation, stomach/abdominal pain, gas, nausea, and vomiting may occur. If any of these effects persist or worsen, tell your doctor or pharmacist promptly.

What are bile acids used for?

In the intestines, bile acids act as detergents and help to emulsify fats, aiding in their digestion and absorption. After participating in digestion in the small bowel, bile acids are almost completely (95%) reabsorbed in the distal ileum and then retaken up from portal blood by the liver (enterohepatic circulation).

What are the two types of resin?

Why do bile acids cause diarrhea?

Bile acids in the large bowel cause abnormally high levels of water and salts to get into the large bowel from the bloodstream. The increased amount of water and salts in the large bowel then causes watery diarrhoea.

Why do bile acid resins cause constipation?

Constipation can be an important side effect of cholestyramine, perhaps because the presence of free bile acids in the intestine helps determine the amount of water in stool, and cholestyramine binds the free bile acids.

Why bile acid resins increase triglycerides?

Activation of phosphatidic acid phosphatase promotes hepatic triglyceride (TG) synthesis, induces secretion of TG-rich, very low density lipoprotein particles, and consequently, increases plasma TG levels.

Does colestipol affect the liver?

Hepatotoxicity. There is little evidence that colestipol causes liver injury. Mild elevations in serum aminotransferase and alkaline phosphatase levels have been reported in small numbers of patients on bile acid resins, although the elevations have been mild, transient and without accompanying symptoms.

Is bile acid malabsorption a disability?

If you find you are having problems with your employer then it is important to be aware that BAD is now recognised as a disability under the Equality Act and employers have a duty to consider reasonable adjustment in the workplace.

What is bile acid diarrhea?

Bile acid diarrhea (BAD) is a condition in which bile acids aren't properly processed by the digestive system. This can cause chronic diarrhea. BAD is also known as bile acid malabsorption (BAM). Malabsorption is an inability of the body to properly absorb certain substances.

What happens if bile acid malabsorption is left untreated?

Untreated bile salt malabsorption can increase your risk of forming gallstones and kidney stones. Vitamin B12 is another vital nutrient which is absorbed into the body at the end of the small bowel.

How serious is bile acid malabsorption?

Bile salt malabsorption may also cause cramp-like pains in your abdomen. These can be very severe. You may also suffer from very smelly wind and very erratic bowel movements. Very occasionally, if too much bile salt is lost, sufferers start to lose weight.

What does malabsorption poop look like?

When there is inadequate absorption of fats in the digestive tract, stool contains excess fat and is light-colored, soft, bulky, greasy, and unusually foul-smelling (such stool is called steatorrhea). The stool may float or stick to the side of the toilet bowl and may be difficult to flush away.

What foods increase bile production?

Bitter foods are great at stimulating bile production. You can choose from all dark green leafy vegetables, as well as beetroot, artichokes and pickles. Drinks such as roasted dandelion root tea, lemon tea, celery juice and coffee all stimulate bile production.
