Treatment and causes of VF is the same as it is for pulseless VT—rapid single defibrillation and chest compressions (see Figs.

Does ventricular fibrillation have a pulse?

A heart monitor that will read the electrical impulses that make your heart beat will show that your heart is beating erratically or not at all. Pulse check. In ventricular fibrillation, there will be no pulse.

Is ventricular fibrillation pulseless electrical activity?

All cardiac arrest rhythms—that is, pulseless rhythms—that fall outside the category of pulseless ventricular tachycardia, ventricular fibrillation, or asystole are considered pulseless electrical activity.

Why is there no pulse in ventricular fibrillation?

Ventricular fibrillation is a type of abnormal heart rhythm (arrhythmia). During ventricular fibrillation, disorganized heart signals cause the lower heart chambers (ventricles) to twitch (quiver) uselessly. As a result, the heart doesn't pump blood to the rest of the body.

Are pulseless ventricular tachycardia and ventricular fibrillation the same thing?

Ventricular fibrillation (VF) and pulseless ventricular tachycardia (VT) are life-threatening cardiac rhythms that result in ineffective ventricular contractions. ... Despite being different pathological phenomena and having different ECG rhythms, the ACLS management of VF and VT are essentially the same.

Pulseless Ventricular Tachycardia

What is the first line treatment for ventricular fibrillation?

Epinephrine is the first drug given and may be repeated every 3 to 5 minutes. If epinephrine is not effective, the next medication in the algorithm is amiodarone 300 mg.

What is pulseless activity?

Pulseless electrical activity (PEA), also known as electromechanical dissociation, is a clinical condition characterized by unresponsiveness and impalpable pulse in the presence of sufficient electrical discharge.

What rhythms are shockable?

The two shockable rhythms are ventricular fibrillation (VF) and pulseless ventricular tachycardia (VT) while the non–shockable rhythms include sinus rhythm (SR), supraventricular tachycardia (SVT), premature ventricualr contraction (PVC), atrial fibrilation (AF) and so on.

How is asystole treated?

Asystole is treated by cardiopulmonary resuscitation (CPR) combined with an intravenous vasopressor such as epinephrine (a.k.a. adrenaline). Sometimes an underlying reversible cause can be detected and treated (the so-called "Hs and Ts", an example of which is hypokalaemia).

What is pulseless VT?

Introduction. Pulseless ventricular tachycardia is a life-threatening cardiac arrhythmia in which coordinated ventricular contractions are replaced by very rapid but ineffective contractions, leading to insufficient organ perfusion and heart failure. Pulseless ventricular tachycardia is a medical emergency.

How do you treat AV fibrillation with a pulse?

1. Cardiopulmonary resuscitation (CPR). CPR mimics the pumping motion of the heart and keeps blood flowing through the body. First call 911 or your local emergency number. ...
2. Defibrillation. This treatment is also called cardioversion.

What are the two non-shockable heart rhythms?

There are two types of nonshockable rhythms, pulseless electrical activity (PEA) and asystole. PEA looks like an organized cardiac rhythm. The electrical activity would normally cause the heart to beat and blood to flow, but in the case of cardiac arrest, each impulse does not generate blood flow.

What are two shockable rhythms?

Shockable rhythms include pulseless ventricular tachycardia or ventricular fibrillation. Nonshockable rhythms include pulseless electrical activity or asystole.

What rhythms are PEA?

PEA encompasses a number of organized cardiac rhythms, including supraventricular rhythms (sinus versus nonsinus) and ventricular rhythms (accelerated idioventricular or escape). The absence of peripheral pulses should not be equated with PEA, as it may be due to severe peripheral vascular disease.

Is asystole a shockable rhythm?

Asystole is a non-shockable rhythm. Therefore, if asystole is noted on the cardiac monitor, no attempt at defibrillation should be made.

Which artery you have to check to see whether he is pulseless or not?

Verify the absence of pulses at the carotid or femoral artery. Epinephrine 1 mg is administered every 5 minutes; vasopressin 40 U can replace the first or second dose.

What is the best treatment for ventricular fibrillation?

External electrical defibrillation remains the most successful treatment for ventricular fibrillation (VF). A shock is delivered to the heart to uniformly and simultaneously depolarize a critical mass of the excitable myocardium.

Why do you give epinephrine for ventricular fibrillation?

Medication Summary

The theoretical benefit of vasopressor medicines, such as epinephrine and vasopressin, is that they increase coronary perfusion pressure. Coronary perfusion pressure is the difference between aortic and right atrial pressure during the relaxation phase of CPR, and it determines myocardial blood flow.

How is Vtach ACLS treated?

Apply defibrillator pads (or paddles) and shock the patient with 120-200 Joules on a biphasic defibrillator or 360 Joules using a monophasic. Continue High Quality CPR for 2 minutes (while others are attempting to establish IV or IO access).

Why is asystole not shockable?

Pulseless electrical activity and asystole or flatlining (3 and 4), in contrast, are non-shockable, so they don't respond to defibrillation. These rhythms indicate that the heart muscle itself is dysfunctional; it has stopped listening to the orders to contract.

How is non-shockable rhythm treated?

1. On recognising asystole, resume chest compressions immediately and continue for two minutes.
2. On recognising organised electrical activity, seek evidence of ROSC and if absent (PEA), resume chest compressions immediately and continue for two minutes.
