The drug has been widely used to treat hypertension globally and is relatively very safe. Hydrochlorothiazide acts on the distal convoluted tubules and inhibits the sodium chloride co-transporter system. This action leads to a diuretic action that lowers blood pressure, but there is also a potassium loss in the urine.

Can hydrochlorothiazide cause low potassium?

Hydrochlorothiazide can lower blood potassium, sodium, and magnesium levels. Low potassium and magnesium levels can lead to abnormalities in heart rhythm, especially in patients already taking digoxin (Lanoxin).

Is hydrochlorothiazide a potassium-sparing diuretic?

Hydrochlorothiazide is a thiazide diuretic (water pill) that helps prevent your body from absorbing too much salt, which can cause fluid retention. Spironolactone is a potassium-sparing diuretic that also prevents your body from absorbing too much salt and keeps your potassium levels from getting too low.

Which diuretic is potassium depleting?

If you take a thiazide diuretic, your potassium level can drop too low (hypokalemia), which can cause life-threatening problems with your heartbeat. If you're on a potassium-sparing diuretic, you can have too much potassium in your blood.

What diuretic does not deplete potassium?

Medications called potassium-sparing diuretics don't lower potassium levels. Examples include spironolactone (Aldactone), eplerenone (Inspra) and triamterene (Dyrenium).

Pharmacology - Diuretics (Loops, Thiazide, Spironolactone) for Registered Nurse RN & PN NCLEX

How much does HCTZ lower potassium?

The high dose produced a mean 0.7 mEq/L reduction in serum potassium while the low dose caused no change.

Does spironolactone increase potassium?

Elevated potassium is a common side effect of spironolactone, but the incidence is undetermined. The likelihood of elevated blood potassium will depend on age, kidney function, other medical conditions, and other drugs being taken.

Is Hydrochlorothiazide a loop diuretic?

Lasix and thiazides are different types of diuretics. Lasix is a type of “loop” diuretic while thiazides refer to a class of diuretics. Lasix is a brand name for furosemide. Examples of thiazide diuretics include chlorthalidone (Thalitone), hydrochlorothiazide (Microzide), and methyclothiazide.

What medications reduce potassium?

• Water pills (diuretics), which rid the body of extra fluids and remove potassium through urine.
• Sodium bicarbonate, which temporarily shifts potassium into body cells.
• Albuterol, which raises blood insulin levels and shifts potassium into body cells.

Should you drink a lot of water when taking hydrochlorothiazide?

Be careful not to become overheated or dehydrated in hot weather while taking hydrochlorothiazide. Talk to your doctor about how much fluid you should be drinking; in some cases drinking too much fluid is just as harmful as not drinking enough fluids.

Should potassium be taken with hydrochlorothiazide?

If you take a low dose of chlorthalidone or hydrochlorothiazide and have normal kidney function, your doctor will probably recommend getting potassium from dietary sources. You can boost your potassium intake from the same foods that are heart healthy - vegetables, fruits and fish.

Why are thiazide diuretics potassium wasting?

Because loop and thiazide diuretics increase sodium delivery to the distal segment of the distal tubule, this increases potassium loss (potentially causing hypokalemia) because the increase in distal tubular sodium concentration stimulates the aldosterone-sensitive sodium pump to increase sodium reabsorption in ...

Why are spironolactone and hydrochlorothiazide given together?

The combination of spironolactone and hydrochlorothiazide is used to treat high blood pressure. This medication is also used to treat patients with edema (fluid retention) caused by various conditions, including heart, liver, or kidney disease.

Does HCTZ raise potassium?

By combining hydrochlorothiazide with triamterene, sodium and water are eliminated from the body without the loss of potassium. However, this drug may allow potassium levels to increase so potassium levels should be closely monitored.

Can HCTZ cause high potassium?

This medicine can raise your blood potassium to dangerous levels, especially if you have kidney disease, diabetes, severe illness, or if you are an older adult. Call your doctor right away if you have signs of high potassium: nausea, tingly feeling, feeling weak or tired, slow heartbeats, or loss of movement.

Does hydrochlorothiazide cause kidney damage?

Popular diuretics include hydrochlorothiazide, furosemide, and spironolactone. They are associated with a risk for acute kidney injury.

What reverses high potassium?

Patients with hyperkalemia and characteristic ECG changes should be given intravenous calcium gluconate. Acutely lower potassium by giving intravenous insulin with glucose, a beta₂ agonist by nebulizer, or both. Total body potassium should usually be lowered with sodium polystyrene sulfonate (Kayexalate).

How much does spironolactone raise potassium?

Spironolactone caused a highly significant increase in the potassium level of 0.5 +/-0.5 mmol/L (p <0.0001) in the entire study population (Table 2).

What is the main cause of high potassium?

The most common cause of high potassium is kidney disease. Other causes of high potassium include: Dehydration. Some medicines.

What is the difference between Lasix and hydrochlorothiazide?

Lasix (Furosemide) is a very effective water pill and it'll certainly make you go to the bathroom more often. Removes water from the body, which lowers blood pressure. Microzide (hydrochlorothiazide (HCTZ)) is a well-tolerated water pill that's often the first-choice treatment for mild high blood pressure.

Which is more effective furosemide or hydroCHLOROthiazide?

Both hydrochlorothiazide and furosemide significantly reduced blood pressure (BP) during three months of therapy. However, the fall in BP was consistently greater with hydrochlorothiazide than with furosemide, although the difference was significant only with respect to systolic BP.

Can a water pill damage your kidneys?

Diuretics. Doctors use these medicines, also known as water pills, to treat high blood pressure and some kinds of swelling. They help your body get rid of extra fluid. But they can sometimes dehydrate you, which can be bad for your kidneys.

Does spironolactone deplete potassium?

Spironolactone is a diuretic that may be used to remove excess fluid (edema) from the body, lower blood pressure, or treat hyperaldosteronism. It does not cause potassium loss from the body; however, other electrolytes may become depleted.

Can spironolactone cause kidney problems?

Spironolactone may worsen kidney function in patients with low to no salt in their body or in those who take other blood pressure medicines (eg, ARB, ACE inhibitor). Talk to your doctor if you have concerns about this.

Can spironolactone cause hyperkalemia?

Hyperkalemia is an established adverse effect of both spironolactone and eplerenone. The symptoms of hyperkalemia begin with muscle weakness. As serum potassium levels rise, symptoms progress to more significant muscle twitching, weakness, nausea, and cramping.
