Autolytic Debridement - This uses the body's own enzymes and moisture to re-hydrate, soften, and liquefy non-viable tissue. Autolytic debridement is selective so that only necrotic tissue is liquefied. Autolytic debridement can be achieved with hydrocolloids, hydrogels and transparent films.

What is autolytic debridement?

Autolytic debridement is the lysis, or breakdown, of damaged tissue at a wound site by the body's natural defence system by enzymes that digest specific components of body tissues or cells, e.g. proteins, fibrin and collagen (Ramundo 2007).

What is the aim of autolytic debridement?

Autolytic debridement is the most commonly used method of debridement. It uses the body's own enzymes and moisture to rehydrate, soften and liquefy devitalised tissue.

What promotes autolytic debridement?

There are dressings specifically designed to promote autolytic debridement, which include thin films, honey, alginates, hydrocolloids, and PMDs. Hydrogels and hydrocolloids are additional dressing choices that may be effective in removing slough.

When is autolytic debridement appropriate?

Autolytic debridement is a conservative approach, often used for patients that cannot tolerate more aggressive forms of debridement. Appropriate application of the moisture-retentive dressing and protection of the periwound skin while using this type of debridement is imperative.

What methods can be used for debridement?

Several types of the debridements can achieve removal of devitalized tissue. These include surgical debridement, biological debridement, enzymatic debridements, and autolytic debridement. This is the most conservative type of debridement.

What is chemical debridement?

Chemical debridement is performed by using certain enzymatic chemicals on the wound that cause lysis of the necrotic tissue in the wound. Commercially available collagenase enzyme granules are sprinkled onto the wound daily until the wound is clear of necrotic tissue.

Is autolytic debridement painful?

3. Autolytic debridement is the slowest method, and it is most commonly used in the long-term care setting. There is no pain with this method. This method uses the body's own enzymes and moisture beneath a dressing, and non-viable tissue becomes liquefied.

How is purulent drainage treated?

If you have purulent discharge or other symptoms of infection, you will need treatment so that it doesn't get worse. Your doctor may need to clean the wound and apply new dressings. They can rinse the site with an antibiotic solution if the infection is small.

Does calcium alginate debride wounds?

This gel formation can also promote autolytic debridement of the wound. Alginates have the unique ability to absorb up to 20 times their weight in fluid, depending on the manufacturer.

Does Slough need to be removed?

Slough is necrotic tissue that needs to be removed from the wound for healing to take place. When referring to slough, some terms may be used interchangeably, fibrotic tissue or necrotic tissue most commonly.

How do you remove slough from wound beds at home?

Wound irrigation, the use of cleansing solutions or a cleansing pad (e.g. Debrisoft®; Activa Healthcare), or the use of dressings – such as hydrogel sheets, honey or iodine cadexomers – can be used to remove slough by clinicians with minimal training.

Can a wound heal without debridement?

Debridement isn't required for all wounds. Typically, it's used for old wounds that aren't healing properly. It's also used for chronic wounds that are infected and getting worse. Debridement is also necessary if you're at risk for developing problems from wound infections.

What is an autolytic wound dressing?

Autolytic debridement is painless and uses dressings to promote moist wound healing. The wound dressing provides a moisture balanced environment that allows the body's natural enzymes to liquify devitalized tissue.

What does the word autolytic mean?

: breakdown of all or part of a cell or tissue by self-produced enzymes.

What is autolytic healing?

Autolysis (i.e. autolytic debridement) uses the body's own enzymatic mechanisms to remove devitalised tissue in order to remove the barriers to healing. This review aims to provide clinicians working in wound care with a better understanding of the mechanisms and implications of autolytic debridement.

Is purulent drainage pus?

Purulent wound drainage is commonly called "pus" and often has a foul or unpleasant smell. Additionally, it can increase an inflammatory response, resulting in intense pain at the wound site and surrounding skin. Wound drainage with a foul odor in and of itself does not indicate infection.

Does purulent drainage smell?

Purulent drainage is yellow, green, brown, or white and has a strong odor.

What does purulent drainage look like?

Purulent drainage is liquid or discharge that oozes from a wound. People usually observe this fluid has a milky look and texture. Purulent drainage is easily spotted, as it is thick and can vary in color, from grayish or yellow to green and even brown.

Why is there a white layer on my wound?

Maceration occurs when skin has been exposed to moisture for too long. A telltale sign of maceration is skin that looks soggy, feels soft, or appears whiter than usual. There may be a white ring around the wound in wounds that are too moist or have exposure to too much drainage.

What happens if necrotic tissue is not removed?

While there is significant disagreement on the correct elocution of the word, the literature is clear that proper debridement is critical to propel wounds toward healing. Necrotic tissue, if left unchecked in a wound bed, prolongs the inflammatory phase of wound healing and can lead to wound infection.

Should I leave dead skin on wound?

When the dead tissue is small, our body can naturally remove it by sending cleaning white blood cells called “macrophages” that produce protein-melting cleaning solutions (proteolytic enzymes). However, large amounts of dead tissue should be removed by other means to prevent infection and facilitate healing.

Is urea used for chemical debridement?

[7] A combination of papain-urea has been used in the debridement of diabetic foot. [6, 8] Papain is a proteolytic enzyme, whereas urea is a chemical agent which denatures nonviable protein.

What are all the agents used for chemical debridement?

The enzyme family

The most common chemicals used for wound debridement are enzymes that digest necrotic tissue, referred to as enzymatic debridement agents (EDAs). Arguably, the most well-known EDA is collagenase (brand name Santyl).
