Most first time MPFL injuries can be treated non-surgically with NSAIDs and immobilization. It's then followed by physical therapy to strengthen the muscles around the knee. Your physician may recommend surgery if the injury is more severe, or if there are small pieces of detached bone or cartilage in the knee.

Can you tear your medial retinaculum?

The MPFL is located on the inner side of the knee and connects the patella to the femur (thighbone). An injury, such as a sprain or tear, to this ligament often occurs because of a forceful, traumatic patella dislocation. MPFL injuries are more common in females and athletes.

Does a torn medial patellofemoral tendon require surgery?

MPFL injuries typically occur during a forceful traumatic kneecap dislocation. This injury is most common among young, active females. Depending on the severity of an MPFL injury, treatment may involve surgical reconstruction, followed by physical therapy.

How long does a torn MPFL take to heal without surgery?

For a first time dislocation, without loose bodies, non-surgical treatment with rehabilitation is recommended. Bracing the joint will allow the swelling to decrease and may help to stabilize the patella. Physical therapy is prescribed, and a full recovery can be expected within an 8-12 week period.

Can a torn MPFL heal on its own?

Left untreated, an injured MPFL can heal on its own. However, when left alone, the ligament heals in a loosened, lengthened position. This causes instability that makes it easier for the patella to become dislocated again in the future.

Does a MPFL tear require surgery?

Most first time MPFL injuries can be treated non-surgically with NSAIDs and immobilization. It's then followed by physical therapy to strengthen the muscles around the knee. Your physician may recommend surgery if the injury is more severe, or if there are small pieces of detached bone or cartilage in the knee.

How do I know if my MPFL is torn?

1. A sense that the knee is buckling and can no longer support your weight.
2. The kneecap slips off to the side of the joint and no longer feels as though it is in the proper position.
3. A catching sensation in the knee joint when you bend or straighten your leg.

Is a MPFL tear painful?

Although most MPFL tears are caused by a traumatic dislocation, some tears can occur without an obvious injury. Symptoms of an injured or torn MPFL may include: Pain in the kneecap, especially when palpated or with activity. Swelling in the knee after activity.

What is the medial Retinaculum of the knee?

The medial patellar retinaculum is a tendon of the knee that crosses the knee joint on the medial side of the patella. ... The medial patellar retinaculum is the branch of the tendon of insertion of the quadriceps femoris that crosses the knee on the medial side of the patella.

Is MPFL surgery painful?

MPFL reconstruction is a painful procedure. Severe postoperative pain can inter- fere with active muscle control. Pain can also impede progress with range of motion (ROM).

What happens if a torn tendon is not repaired?

If left untreated, eventually it can result in other foot and leg problems, such as inflammation and pain in the ligaments in the soles of your foot (plantar faciitis), tendinitis in other parts of your foot, shin splints, pain in your ankles, knees and hips and, in severe cases, arthritis in your foot.

Can you walk with a torn tendon in your knee?

If the tendon is torn, it can become difficult to walk or carry out other everyday activities. If you have a serious tear, you will probably need to have surgery to regain your full range of movement.

How serious is a patellar tendon rupture?

Small tendon tears can make it difficult to walk and participate in other daily activities. A large tear of the patellar tendon is a disabling injury. It usually requires surgery and physical therapy afterward to regain full knee function.

What does the Retinaculum do?

A retinaculum (plural retinacula) is a band of thickened deep fascia around tendons that holds them in place. ... Its function is mostly to stabilize a tendon. The term retinaculum is New Latin, derived from the Latin verb retinere (to retain).

Where does a MCL tear hurt?

MCL injuries hurt. Most people feel pain along the inside edge of the knee, and they also have swelling. You might hear a pop when the damage to the knee takes place, and your knee may lurch to the side. You may find it hard to walk, or feel like you can't put pressure on the leg with the hurt knee.

What is a Retinacular tear?

Medial and lateral retinacular tears occur when there is valgus or varus angulation while the quadriceps is contracted.

What happens if you tear your retinaculum?

When functioning correctly, tendons glide under these retinacula without hindrance. With injury (trauma, repetitive strain), the retinaculum can become a site of tendon restriction, nerve impingement, and circulatory compression. Injury to the retinaculum will cause mechanical and neurological damage.

What does Mpfl reconstruction stand for?

• MPFL stands for Medial Patellofemoral Ligament. This is a structure that secures the kneecap. and helps prevent it from dislocating to the outside of the knee. • The MPFL connects the kneecap to the inside part of your knee at the thigh bone (femur).

Is the Mpfl the same as the medial retinaculum?

It is located superior and anterior to the medial collateral ligament and is part of the medial patellar retinaculum and the intermediate medial capsular layer ^1-4 and it forms the medial patellofemoral complex together with the medial quadriceps tendon femoral ligament ⁵.

When is MPFL surgery necessary?

MPFL surgery is necessary for a kneecap which keeps dislocating. An unstable kneecap may result in arthritis of the kneecap (patellofemoral arthritis) in later years. This is because each time the patella dislocates, the smooth cartilage lining the kneecap may be damaged.

What happens when you tear your MPFL?

Dislocation also occurs when the MPFL is torn. The common symptoms include pain, tenderness, swelling around the knee joint, restricted movement of the knee, numbness below the knee, and discoloration of the area where the injury has occurred.

How successful is MPFL surgery?

Modern MPFL reconstruction is a well-described operative intervention which is successful in the vast majority of chronic patellar instability patients. Excellent functional results have been reported as high as 95%, and numerous studies have documented significant reduction in postoperative re-dislocation [18, 19].

Can you dislocate your knee without tearing ligaments?

Causes. Patellar dislocations can occur either in contact or non-contact situations. An athlete can dislocate his/her patella when the foot is planted and a rapid change of direction or twisting occurs. Usually a pre-existence ligamentous laxity is required to allow a dislocation to occur in this manner.

What is medial collateral ligament injury?

An MCL injury is a sprain or tear to the medial collateral ligament. The MCL is a band of tissue on the inside of your knee. It connects your thighbone to the bone of your lower leg. The MCL keeps the knee from bending inward.

How do you strengthen the medial patellofemoral ligament?

Early exercises include quadriceps sets, heel slides, hamstring sets and gluteal sets until the patient is full weight bearing without symptoms. Because the hip and trunk are so important in maintaining proximal control for the knee and the patellofemoral joint, total leg strengthening (TLS) is initiated early.
