Other dopamine antagonists dopamine antagonists Medications with central dopamine antagonist properties are in wide use in treating a variety of medical symptoms. Some of the most commonly used are metoclopramide (Reglan), prochlorperazine (Compazine), droperidol (Inapsine), and promethazine (Phenergan). › ...

Adverse neuropsychiatric effects of dopamine antagonist medications ...

including metoclopramide, chlorpromazine and prochlorperazine are associated with QT prolongation,⁴ but the minimum dose that causes ECG changes is unknown.

Does prochlorperazine prolong QT?

Prochlorperazine may increase the risk of QT interval prolongation. Best to avoid use with other potentially QT-prolonging medications. Combination of prochlorperazine with SSRI/SNRI, ondansetron, metoclopramide, promethazine, isoflurane, and sevoflurane may increase the risk of QT prolongation.

Which antiemetics cause QT prolongation?

Chlorpromazine (THORAZINE) and promethazine (PHENERGAN) may prolong the QT interval.

Which antiemetics do not prolong QT?

Scopolamine is an antimuscarinic mediation with a similar anticholinergic side effect profile as antihistamines. However, by acting as an M1-muscarinic antagonist, it is non-sedating and does not prolong the QTc. Its use is limited to motion sickness prophylaxis and it is only available as a transdermal patch (10).

Does Albuterol cause prolonged QT interval?

In addition, studies have shown that the significant extrapulmonary effects of inhaled albuterol, which include increased heart rate [9,24,26,27], increased QT interval [26] and decreased plasma potassium level [9,24,26,28] can all occur within the maximum recommended dose.

Does Remeron cause QT prolongation?

During the postmarketing use of mirtazapine, cases of QT prolongation, Torsades de Pointes, ventricular tachycardia, and sudden death, have been reported (see ADVERSE REACTIONS).

Does Cyclizine prolong QT?

Cyclizine would be the anti-emetic of choice in this patient as it has not been associated with QT prolongation.

Does Zofran prolong QT?

ECG changes including QT interval prolongation have been observed in patients receiving ondansetron. In addition, Torsade de Pointes, an abnormal, potentially fatal, heart rhythm, has been reported in some patients receiving ondansetron. The use of a single 32 mg intravenous dose of ondansetron should be avoided.

Does Thorazine cause QT prolongation?

This is consistent with results of other published reports. Chlorpromazine is considered a high-risk drug for QTc prolongation, with one study finding an increased relative risk of 1.37 (95% CI 1.14-1.64).

Is Compazine and Phenergan the same?

The brand names Compazine and Phenergan are discontinued in the U.S. Generic versions may be available. Compazine and Phenergan belong to different drug classes. Compazine is a phenothiazine anti-psychotic and Phenergan is an antihistamine.

Can compazine cause seizures?

Get medical help right away if you have any very serious side effects, including: seizures. This medication may rarely cause a very serious condition called neuroleptic malignant syndrome (NMS).

When should QT prolongation be considered in antiemetic use?

QTc prolongation from routine doses of 4–8 mg of ondansetron is minimal and is unlikely to be clinically significant or cause a dangerous arrhythmia. A patient who receives a single dose of an antiemetic does not require routine screening electrolytes or an electrocardiogram (ECG).

Which drugs prolong QT the most?

Drugs that cause QTc prolongation

A long QT interval is most frequently seen with class I and class III antiarrhythmic drugs. Other classes of drugs that cause QTc prolongation include antihistamines, antidepressants, antibiotics, antifungal drugs and antipsychotics (Table 2).

What medications cause torsades Depointes?

Other drugs that prolong the QT interval and have been implicated in cases of torsade include phenothiazines, tricyclic antidepressants, lithium carbonate, ziprasidone, cisapride, highly active antiretroviral drugs, high-dose methadone, anthracycline chemotherapeutic agents (eg, doxorubicin, daunomycin), some ...

Why does Zofran cause prolonged QT?

Based on animal studies, 5-HT₃ antagonists appear to prolong the QT interval by blocking I_Kr channels. In addition, these drugs block the cardiac sodium channels leading to QRS interval widening, which consequently prolongs cardiac depolarization and further lengthens the QT interval and can subsequently lead to TdP.

Does oral Zofran prolong QT interval?

- ECG changes including QT interval prolongation have been seen in patients receiving Zofran (ondansetron). In addition, Torsade de Pointes, an abnormal heart rhythm, has been reported in some patients receiving ondansetron.

Why is Zofran contraindicated in prolonged QT?

The drug may increase the risk of prolongation of the QT interval of the electrocardiogram, which can lead to abnormal and potentially fatal arrhythmias, including torsade de pointes, according to an FDA press release.

Does Flexeril cause QT prolongation?

Cyclobenzaprine is structurally related to the tricyclic antidepressants, which have been reported to cause tachycardia, arrhythmias, heart block, hypertension, hypotension (particularly orthostatic hypotension), thrombosis, thrombophlebitis, myocardial infarction, strokes, congestive heart failure, and ECG ...

Does azithromycin cause QT prolongation?

Azithromycin exposure has been reported to increase the risk of QT prolongation and cardiovascular death. However, findings on the association between azithromycin and cardiovascular death are controversial, and azithromycin is still used in actual practice.

Does Decadron prolong QT?

Treatment with high-dose dexamethasone was repeatedly associated with a decrease in the QT interval that was reversible upon cessation of treatment.

Why is mirtazapine less sedating at higher doses?

However, at low doses (e.g. - 7.5 mg), mirtazapine has a higher affinity to (and thus preferentially blocks) the histamine-1 receptor, compared to the other 2 receptors. At higher doses, this antihistamine activity is offset by increased noradrenergic transmission, which reduces its sedating effects.

Does mirtazapine cause tachycardia?

Do all SSRIs cause QT prolongation?

Case reports of QT prolongation have been published with all of the SSRIs, with the exception of paroxetine, but many of these events were in the context of an overdose, inherent risk factors, and concomitant use of other medications known to increase the QT interval, which makes it difficult to draw conclusions.
